At the Salt Lake City VA Healthcare System we offer a 1 year postdoctoral fellowship in Geropsychology. Our internship program also receives funding from our GRECC (Geriatric Research Education and Clinical Center), so we offer internship rotations that support our generalist training in geropsychology. Geropsychology related internship rotations that we offer are Homebased Primary Care and the Geri-Med-Psych clinic. Each of these professional training experiences is described below (interns work in the clinics described below with our fellows):

1. Geopsychology Fellowship Track
This track provides general clinical psychology fellowship training, with an emphasis in geriatrics. Broadly, the geriatric training provided is consistent with the Pikes Peak Model for Training in Professional Geropsychology. Specific geriatric emphases include: 1) evidence-based assessment and treatment of psychological disorders among older adults; 2) biopsychosocial methods for managing disruptive behaviors associated with dementia; 3) neuropsychological assessment and dementia management; 4) behavioral medicine interventions addressing common problems among older adults, such as pain, disability, sleep, weight loss, smoking cessation, and medical compliance; 5) interdisciplinary consultation; 6) capacity assessments when there are questions about a patient’s ability to make medical decisions, perform other specific functions, or live independently; 7) development of clinical supervision competencies; and 8) identification of, understanding, and addressing ethical issues common among geriatric populations.
Fellows will provide psychological services across a variety of diverse clinical settings, including Home Based Primary Care (HBPC), outpatient geriatric primary care clinics, specialty geropsychiatry-medical clinic, primary care, and palliative care. Fellows will also gain experience working with multidisciplinary programs, including the Geriatric Research, Education, and Clinical Center (GRECC), the Office of Rural Health—Western Region, and the University of Utah, Division of Geriatrics. It is also anticipated that fellows will cross-train in some of the same training experiences as the health psychology fellows.
Fellows have the opportunity to participate actively as members of interdisciplinary teams in HBPC, Geriatric Primary Care clinics, the Clinical Resource HUB, and the Telehealth Dementia Care Consult Team. Rotations will be tailored to fit the individual needs and preferences of the fellow. Rotations may consist of a combination of geriatric experiences (approximately 75% of the fellow’s time) including Home Based Primary Care, outpatient geriatric clinics, Geriatric Medical Psychiatry clinic, or VISN 19 Clinical Resource HUB (an entirely telehealth-based rotation). Fellows will also have the opportunity to develop expertise in complementary areas of health psychology. Minor rotation experiences (approximately 25% of fellows time) may include primary care-mental health integration (PC-MHI), short-term therapy in primary care, MOVE program, smoking cessations program, inpatient rehabilitation, consultation and training for PACT team members, MH Consult, CBT for Chronic Pain or Insomnia, , and transplant and bariatric surgery evaluations (see Health Psychology section for descriptions). Fellows will select opportunities offered from the areas/supervisors listed above. Experiences can be designated as year-long or time limited.

GEROPSYCHOLOGY FELLOWSHIP ROTATIONS
HOME BASED PRIMARY CARE (HBPC)
Supervisor: Lauren Masuda, PsyD
The VA Home Based Primary Care (HBPC) program provides comprehensive, interdisciplinary primary care services in the homes of veterans with complex and chronic, disabling disease. In addition to primary care interventions, HBPC provides palliative care, rehabilitation, disease management, and care coordination services. HBPC targets veterans in advanced stages of chronic disease, particularly those at high risk of recurrent hospitalization or nursing home placement. Outcome measures have demonstrated HBPC to be effective in managing chronic disease and reducing inpatient days and total cost of care. Our HBPC team includes representatives from medicine, nursing, pharmacy, social work, occupational therapy, and dietetics. HBPC manages (1) patients with multiple interacting chronic medical problems requiring longitudinal intervention to maintain health status or slow functional decline, in order to reduce or delay institutionalization; (2) certain patients with relatively short-term problems, who need health services, home training, and home adaptation until they can be managed in an outpatient clinic; and (3) patients with advanced terminal illness who prefer palliative care. HBPC provides a unique opportunity for fellows to be part of a diverse team with opportunities to work collaboratively, co-treat clients when appropriate, foster effective communication and teamwork amongst providers, and help the team to cope with various challenges.
The postdoctoral fellow provides assessment and intervention to individuals and families to address psychological issues that are interfering with their medical care, compromising their health status and functional capacity, and/or reducing their quality of life. Issues include, but are not limited to: helping patients cope with grief, depression, anxiety, and other psychological issues related to the aging process; coping with chronic medical illnesses, and/or other life stressors; interventions to increase compliance with and adjustment to treatment regimens; and working with caregivers to improve patient well-being and treatment compliance. The postdoctoral fellow also provides comprehensive psychological, neuropsychological, and capacity evaluations.

OUTPATIENT GERIATRIC CLINICS
Supervisor: Bret Hicken, PhD
These weekly primary care clinics serve older Veterans with complex medical issues and are staffed by geriatric fellows, geriatricians, clinical pharmacists, and social workers, in addition to the psychology fellow. Veterans in these clinics frequently have co-morbid cognitive and mental-health issues as well as psychosocial challenges that complicate their care. Psychology fellows serve in a consult/liaison role in these clinics, consulting with the team and providing brief psychological and cognitive assessments during the appointment and Fellows may also provide instruction to staff on psychological issues. Time-limited interventions may also be incorporated during clinic or as follow up. Common issues that psychology fellows may address with Veterans include depression, anxiety, grief, pain management, sleep/insomnia, and adjustment to aging and chronic disease.

GERIATRIC MEDICAL PSYCHIATRY CLINIC

This weekly consultation clinic serves geriatric Veterans with complicated medical and psychiatric needs. Veterans referred to this clinic typically have significant mental health and/or cognitive issues and disruptive behaviors that have become difficult to manage for caregivers and providers. The philosophy of this clinic is “patient-centered” and “whole-person” in that we address the psychosocial, environmental, and medical factors that influence behavior. In addition to the psychology fellow, the clinic includes an attending physician and trainees from many disciplines such as psychiatry, geriatrics, and pharmacy. Psychology fellows function in a variety of capacities in this clinic. They may provide direct patient care in the form of brief psychological and cognitive assessments as part of the patient’s clinic appointment. They may provide brief intervention in clinic or arrange for a later appointment to address clinical concerns that require follow-up. Trainees also function in a consultation capacity by providing the team with observations, impressions, and treatment recommendations about psychological and cognitive issues. Over time, the fellow becomes a key leader in clinic, directing and providing training to housestaff and medical students about psychological issues.

VISN 19 CLINICAL RESOURCE HUB

The VISN 19 Clinical Resource Hub provides evidence-based assessment and treatment services to facilities in the VA Rocky Mountain Network (UT, MT, CO & WY) in order to enhance Veteran access to mental health and primary care services. The Telemental Health Team is comprised of psychologists, psychiatrists, advanced psychiatric nurse practitioners, and social workers that assist VA facilities across VISN 19 with provision of virtual mental health services.
Training opportunities for fellows on this rotation include:
• Dementia Caregiver Education & Support: Fellows have the opportunity to provide evidence-based caregiver support virtually to caregivers caring for rural Veteran’s with a neurocognitive disorder. Services include education on dementia, behavioral interventions for managing disruptive behaviors associated with dementia and stress management techniques. Fellows may have the opportunity to complete the REACH-VA Caregiver Support training and implement it during this rotation.
o Telehealth Dementia Care Consult Team: Fellows also have the opportunity to attend consults with the interdisciplinary team (geriatrician, geriatric social worker, pharmacist) that provides comprehensive evaluation of neurocognitive disorders to older rural Veterans. This will allow the opportunity to introduce caregiver support and gain a better understanding of each member’s role on the team.
• Geriatric Tele-Psychotherapy:
o Outpatient Geriatric Tele-Mental Health: Fellows have the opportunity to carry a small caseload of older Veterans referred for evidence-based individual psychotherapy delivered via VA Video Connect to Home or via Clinical Video Telehealth to VA Clinic. Patients present with a variety of primary concerns, including depression, anxiety, PTSD, grief and adjustment to disability. Fellows can hone their skills at adapting evidence-based treatments to meet the needs of older adults and learn how to effectively implement treatment via telehealth.
o Tele-PCMHI & Health Psychology: Fellows have an opportunity to receive geriatric consults directly from primary care providers (in general PACTs across CO & UT) pertaining to a variety of concerns, including cognitive functioning, chronic disease management, adjustment to medical illness, depression, alcohol use, sleep disturbances, and dementia care management. Fellows can refine their skills in brief assessment, consultation, and care management around normative and non-normative age-related concerns that arise in primary care. They can also learn to deliver geriatric-informed brief psychotherapy and adapt evidence-based interventions to meet the needs of older adults in primary care via telehealth. There may be opportunities to develop and/or facilitate group interventions aimed at meeting the unique needs of older adults or their caregivers within primary care. Fellows may also have an opportunity to provide vertical clinical supervision to a psychology intern while on this rotation.

Faculty Clinical/Research Interests:

Kristy Shoji, Ph.D., ABPP (she/her/hers) is a board certified Clinical Geropsychologist working in geriatric primary care and providing consultation services to contract nursing homes. She obtained a master’s degree from Wake Forest University and doctorate degree from the University of Alabama – Tuscaloosa with a concentration in geropsychology. She then completed a geropsychology internship and fellowship at South Texas Veteran’s Health Care System. She has been involved with training in the VA for the past six years as a preceptor, supervisor, and assistant training director. Clinical areas of interest include neuropsychological and capacity evaluation, behavioral interventions/caregiver support for challenging behavior in the context of dementia, and end of life intervention. Dr. Shoji is the Primary Supervisor for the Geropsychology Residency and a member of the Postdoctoral Training Committee. Outside of work, Dr. Shoji loves being a mother and partner, spending time with friends and family, cooking, crafting, reading, and taking barre classes.

Katie Johanson, PhD (she/her/hers) is a clinical psychologist in Home-Based Primary Care. She completed her doctoral training in Clinical Geropsychology at the University of Colorado Colorado Springs, followed by an internship in Clinical Health Psychology at the Memphis VA and a fellowship in Geropsychology at the VA Salt Lake City Health Care System. Her clinical interests include CBT-based interventions (including ACT and PST) for mood and anxiety conditions; CBT- and ACT-based treatments for insomnia (e.g., ABC-I), including adapting interventions to special populations; adjustment/end-of-life transitions; decision-making capacity; and caregiver support with a whole-person approach. With respect to research, Katie continues to recover from her dissertation but enjoys studying insomnia/diversity in sleep problems, group interventions, and participating in interdisciplinary quality improvement projects. She is the secondary supervisor for the geropsychology residency, a member of the postdoctoral training committee, and Co-Chair of the ECHCS Dementia Steering Committee. Katie fills her free time with trail running, exploring the city with her partner, reading Scandinavian crime thrillers in the company of her cats, and pressuring friends to share an order of every dessert on the menu.

Faculty Clinical/Research Interests:

At the fellowship level, fellows complete two 6-month rotation during the training year across all of the above settings. In addition to direct clinical training experiences, as part of their professional development as psychologists, all fellows will have the opportunity to provide supervision to psychology doctoral interns, under the supervision of a staff psychologist. Fellows will be required to provide one hour of supervision per week to an intern. In addition, fellows attend the Supervisor Development Series, a program designed for the staff psychologists to discuss supervisory issues and refine supervisory skills, and receive supervision training during the Professional Development Seminar series (see below). Furthermore, fellows will attend the monthly supervisor meetings and will serve on the Clinical Training Committee by attending quarterly meetings and assisting with the selection of interns and fellows.  Fellows receive a minimum of two hours of formal individual supervision weekly. Supervisors are also available on an as needed basis beyond the regularly scheduled times of supervision. Progress towards the attainment of the training goals is determined by means of regularly scheduled evaluations. Feedback between fellows and supervisors is on going. Monthly evaluations are discussed informally with the fellow and at a monthly supervisors’ meeting chaired by the Director of Clinical Training. A mid-rotation evaluation occurs at the 3-month and 9-month time points with the supervisor reviewing the fellow’s progress thus far in terms of the specific skills defined within each core competency area. At the conclusion of each six-month rotation, a formal evaluation of the fellow’s progress is made.  Didactic training offerings your organization: As part of the GRECC rotation, Geropsychology interns and fellows participate in an interdisciplinary educational series, which includes a case conference and core lecture series featuring topics crucial for providing clinical services for the older adult population. This educational series provides an excellent opportunity to interact with trainees from other disciplines with a shared interest in providing comprehensive clinical services to older adults. Other offered didactics for both interns and fellows include monthly Geropsychology journal club, Neurobehavioral case conference, and Neuroanatomy seminar series.  Geropsychology fellows attend the above noted didactics as well as the Geropsychology Fellowship Diversity Seminar which is a series of five seminars designed for geropsychology fellows that are held during the year. These seminars focus on diversity issues in the assessment and treatment of older adults, such as assessment considerations with Black older adults, diversity in long-term care, and considerations in psychotherapy with LGBT older adults. The Geropsychology fellowship staff supervisors serve as the presenters for these seminars. In addition, The Professional Geropsychology fellows participate in a 1-hour weekly Fellowship Seminar Series with the 3 clinical psychology fellows. This seminar series includes a Professional Development Seminar, Fellowship Case Conference, Fellowship Seminar, Supervisor Development Series, and Diversity Seminar.

Training Program Components: 
Training Levels and Rotations: We offer geropsychology training exposure and experience for interns (in a general health service psychology program) and for a geropsychology fellow. We offer these trainings through our four geriatric programs at the Hines VA: Home Based Primary Care, Geri-PACT (geriatric primary care), Palliative Care, and in our Community Living Center (consisting of long-term care, hospice, subacute rehabilitation, and skilled nursing). Psychology interns may select rotations that fit their training needs, with rotations lasting 6 months for 50% of their time. Our geropsychology fellow selects 3 of the 4 rotations and spends 50% of their time in one rotation (the Major Rotation) all year and 50% of their time in the two others (Minor Rotations) for half a year each.

Supervision:
As an APA-accredited program, our interns and fellows are provided with 4 hours a week of supervision with our training director and rotation supervisors. We have 4 rotation supervisors who primarily work with older adults (one of whom has completed a formal fellowship in geropsychology and is board-certified in geropsychology). We also collaborate frequently with all available disciplines, including RNs, NPs, MDs (primary care, palliative care, and psychiatry), geriatrics-focused pharmacists, dietitians, social workers, and physical therapists, all of whom offer content-specific guidance in their specialty areas that help to enhance the knowledge that trainees gain while they are on our training rotations.

Case Consultation:
In addition to formal supervision and curbside consultation with direct supervisors, we hold a once-weekly “Geri Squad” meeting, which is comprised of Psychology and Psychiatry providers in the geriatric clinics. During these weekly meetings, trainees are offered the opportunity to present challenging or interesting clinical cases and receive feedback from other members of our team. In addition, interns and fellows at Hines are part of a monthly Integrated Care Grand Rounds, a series dedicated to various aspects of interdisciplinary care. In this setting, too, trainees receive consultation. Lastly, our Diversity Case Conference series allows for trainees to present on cases/issues that relate to diversity and receive feedback from a broader group of staff psychologists.

Experiential Workshops:
Each of our trainees attends a Goals of Care Conversation training workshop led by a nationally-trained coordinator for our local Life Sustaining Treatment Decisions Initiative. Trainees may also complete the REACH-VA (Resources of Enhancing All Caregivers Health) workshop.

Additional Trainings:
In addition to the above offerings coordinated locally, all interns and fellows are provided with a monthly list of VA-wide, and when available community-specific, geriatric and geriatric mental health care trainings. For example, the “Meeting the Mental Health Needs of Aging Veterans: Promising Practices” series. Trainees select which ones they attend in line with their interests and goals, but these do span the continuum of topics. Some recent ones have included Mobility in Older Adults, End-of-Life Care in the Time of COVID-19, and Cultural Formulation Interview, Ethnicity, and Older Veterans.

Experiences and Competencies Training: Our training experiences at the internship and fellowship level are designed with an emphasis on exposure, experience, and developing expertise on the application of psychological science to care of older adults. Our core training faculty actively consider the application of the Pikes Peak model and other professional recommendations on designing training programs (e.g., Hinrichsen et al., 2018). As noted above, our program relies on our core geropsychology training faculty, local mental health and interdisciplinary partners, as well as national trainings offered within the VA System to reach these aims.

Across these trainings, our trainees reflect on their attitudes about older adults and aging, gain general knowledge about adult developing and aging process (particular focus on health and functioning), learn about basic of psychological practice with older adults, knowledge of core assessment techniques and challenges, and a variety of evidence-based interventions. Development of competencies are tracked via the Pikes Peak Evaluation Tool. Trainees complete this measure at the start of their rotations and again at the end. Supervision includes attention to developing areas where additional knowledge or skill is needed, and if needed, this is coordinated via the core training faculty to ensure competencies are developing.

Training Program Components: 
Pre-doctoral internship trainees have the opportunity to provide clinical services and receive supervision in the VA Community Living Center, Hospice and Palliative Care, and Neuropsychology Clinic. These settings serve primarily older veterans. Beginning with the 2019-2020 training year, interns will also have the opportunity to complete a rotation with the Home Based Primary Care Program. All training settings emphasize the development of knowledge, skills and attitudes consistent with the Pike’s Peak Model.

Starting with 2017-2018 training year, Durham VAHCS received additional funding for a Fellowship position with emphasis in Clinical Geropsychology. Training experiences within the Durham VA Health Care System include Community Living Center (Rehabilitation & Long-Term Care), Palliative and Hospice Care, Neuropsychology Clinic, Home Based Primary Care Program, and Late-Life PTSD & General Outpatient Mental Health. Setting were chosen to provide trainees with the opportunity to hone their clinical skills in the major core competencies defined in the Pike’s Peak Model for Geropsychology Training (Theoretical/Conceptualization Skills, Psychological Assessment, Psychological Intervention, Consultation, Program Development and Evaluation, Clinical Supervision, and Teaching). Fellows receive advanced training in selecting, adapting, and implementing assessments and evidence-based interventions for older adults and their supports. Interdisciplinary teamwork and provision of consultation to other healthcare professionals are also emphasized across training settings.

Didactic Training Offerings:
Durham VAHCS Psychology Training Program holds a monthly Geropsychology Journal Club through which all geropsychology competencies are discussed. Intern attendance is optional, but Fellows are required to attend. Fellows are also required to lead at least 2 discussions in journal club. Interns also receive didactics through their seminar series on topics such as Providing Culturally Competent Care to Older Veterans, Elder Abuse, and Hospice and Palliative Care. All trainees are also encouraged to participate in geropsychology-relevant webinars, including the VA’s monthly “Meeting the Mental Health of Aging Veterans” and “Decision Making Capacity” webinar series.

Faculty Clinical/Research Interests: The NF/SG VHS offers a GRECC geropsychology track fellowship residency with training experiences at two locations. In Gainesville, the Community Living Center offers short-term rehabilitation/skilled nursing, Geriatric Evaluation and Management (GEM), and palliative care consult team. In Lake City, the Community Living Center offers short-term rehabilitation/skilled nursing, specialty dementia care, long-term nursing care, and hospice/palliative care (inpatient and consult team). In the CLCs, clinical experiences include individual psychotherapy, group psychotherapy, interdisciplinary team collaboration, medical rounds, care planning, cognitive and capacity evaluations, and behavioral management utilizing STAR-VA. In addition to clinical experiences, trainees participate in culture transformation, Montessori-Based dementia programming, and staff trainings. Other available training experiences include Home Based Primary Care, Geriatric Primary Care, and Neuropsychology Clinic. Residents also participate in supervising interns and practicum students.

Lisa Bloom-Charette, Ph.D, is a staff psychologist is a specialist in clinical gerontology in the Community Living Centers . She is also on the faculty at the Boston University School of Medicine. Her clinical and research interests include substance abuse in the elderly, life review; code decision ethics, long term care teams, and helping staff deal with resident’s difficult behaviors using STAR-VA. She is the co-editor of the book, Enhancing the Quality of Life in Advanced Dementia and serves on both VA and national Geropsychology committees.

Kristen Dillon, Psy.D. is a staff psychologist in Hospice & Palliative Care and primary psychologist on one of the Community Living Centers. Her research and clinical interests include anticipatory grief, caregiving, bereavement, serious mental illness in the elderly, culture change and interactions between staff and Veterans.

Matthew Wachen, Ph.D. is a staff psychologist in Home Based Primary Care. His interests include geropsychology, the integration of mental health and primary care, and the management of chronic disease and maladaptive behaviors with cognitive behavioral therapy and mindfulness-based techniques.

Brian Zuzelo, Psy.D. is a staff psychologist in home based primary care services providing psychotherapy and assessment services with veterans in the community. Faculty Clinical/Research Interests:
The mission of the Sierra Pacific Mental Illness Research Education and Clinical Center (SP-MIRECC) is to build an integrated system of clinical, research, and educational efforts designed to improve the clinical care for veterans with dementias and with PTSD both in VISN 21 and system-wide. Dementia and PTSD share common clinical symptoms including cognitive difficulties, sleep disorders, and agitation and the SP-MIRECC aims to evaluate current approaches and develop new treatments for these clinical problems. In support of this mission, the SP-MIRECC Fellowship provides specialized training in two emphasis areas: Aging/Dementia and PTSD.  Fellows devote the majority of their time to patient oriented research and education activities and 25% to direct patient clinical care. In collaboration with their mentors, Fellows will develop and implement a research project, publish and present findings, participate in grant writing, and utilize the latest technology for educational activities and clinical service delivery.
